Regular Appointments

Appointments with your doctor can be booked online here , or by calling 519-658-4615 to speak with our reception team.

You can also book online with our nurse for paps, injections, ear syringing, and other procedures.

After Hours

If you experience an urgent medical issue and need to see a physician after hours, please call 519-658-4615. These calls are answered M-Th until 8 pm, and Saturday from 10-1 pm.

To speak with a nurse after 7 pm, please call 1-866-553-7205. This line is answered from 5 pm to 9 am M-F and 24 hours on weekends. The nurse will have access to our physician on call for further directives if necessary.

Insurance Forms

To submit paper work for your physician to complete, please email a copy of your form along with any necessary accompanying info to forbesparkmed@gmail.com. Please allow 14 day turn around for paper work completion.

No Shows

There is a no show fee for $40 for any missed office visits without a 24 hour cancellation notice.
